Inquest touching the death of Thomas Brookes

HM Senior Assistant Coroner for Sunderland Karin Welsh has concluded the Inquest touching the death of Thomas Brookes. Thomas was 16 years old and placed at home under a Care Order at the time of his death in September 2019. The Coroner’s conclusion was one of death by misadventure, having been unable to establish a causal link between identified failings on the part of Together for Children Sunderland and the death.
